two reasons to treat - decrease symptoms, increase longevity
PVCs without a history of heart disease (35%) - not threating
PVCs with a history of heart disease
- at risk for sudden death? (5%) -> implantable defibrillator
- no good evidence that drug therapy increases length of life
- resist starting medications unless needed to control patient's symptoms
treatment of choice may be beta-blockers
holter monitor anytime the patient has symptoms
